Divorce Public Record

Can you please tell me where I may find and or buy
a Praecipe to transmit form? I have filled out my own
divorce papers and submitted them with payment but
have been informed they will not go before the Judge
until I submit the Praecipe To Transmit Record form for Western wayne PA
this did not come with the kit I used to draw up my
divorce papers and I can not find the form any place.
Please advise, any assistance in this matter will be
gratefully appreciated
